Your First Appointment

Your first appointment will consist of a medical history interview, physical examination and possibly allergy testing and/or pulmonary function testing. At the conclusion of the evaluation, Dr. Bajowala will review with you your personalized treatment plan and submit any necessary prescriptions to your pharmacy. Please budget 60-90 minutes for your first appointment.

By completing your initial paperwork online using our Patient Portal, you won't have to spend time filling out forms when you should be spending time with the doctor! Online history submission enables Dr. Bajowala to review your history before your appointment, so she can give you her undivided attention.

Please read these Important Instructions in Preparation for Skin Testing - it will explain our testing methods, what to expect, and medications to hold before your first visit.

Please also read our updated Notice of Privacy Practices - a paper copy will be made available to you upon request.

Insurance Network Participation

Kaneland Allergy & Asthma Center participates in many major insurance plans- click here for an updated list of the insurance networks we participate in. If you don't see your plan listed, please contact your insurance company to verify our participation. All insurance referrals are the patient’s responsibility. Our office bills primary insurance; filing of secondary insurance is the patient’s responsibility.

Please bring your insurance card and a photo identification card with you on the day of your appointment. Without these items, our office will be unable to bill your insurance directly.
Electronic Health Records

Kaneland Allergy & Asthma Center uses electronic medical records and e-prescribing.

In addition to helping the environment, electronic health records are convenient and secure. EHR's also facilitate communication between various healthcare providers, keeping everyone involved in your health on the same page. Electronic prescribing allows your prescriptions to be ready faster, saves paper, and reduces the risk of dangerous drug interactions.

Patient Portal

Our advanced patient portal lets patients conveniently complete forms, schedule appointments, securely message the physician and pay bills online. Once your first appointment is scheduled, you'll receive a login and password for our patient portal, as well as a convenient link to complete your medical history online.
